What is Pathway of Hope?
- A strengths-based case management program designed o reach families with school-aged children (Pre-K - High school).
- A holistic focus on individual families who desire to overcome their barriers and improve stability.
- The overarching goal is to break the cycle of intergenerational poverty.
- Focused on long-term transformational change (rather than transactional services).

Eligibility Requirements
- Family with school-aged children in the home.
- Must be willing and ready to make a change in their lives.

Additional Information
- NO COST to joining the program.
- Assistance with setting goals and action steps to achieve the goals.
- Accountability partner
- Connection to resources in the community
