Call the local Social Security administrative office to make sure the initial application has been received and that a number has been established. The safest way to know for sure is to wait until the baby is at least two months old. That gives Social Security time to process that original application.

Once you know the number has been issued, follow this link to the Form SS-5: Form SS-5.

This must be sent in with a form of ID for the baby. The representative I spoke to did not recommend a driver’s license or birth certificate as it is a major inconvenience to be without these documents. Instead, she recommended obtaining a form of ID like a medical record for Khalil or immunization. So long as the clinic or hospital has stamped and dated the document. Also ID for mom can be hospital records that have her, name, DOB, and patient ID, as well as the stamp and date. Can’t use discharge papers.

On top write “Never received Social Security card after birth.” That way we know why she is not providing. Applying for housing would be reason right away and the service coordinator can add their name, agency, and contact information.
