Live Well by Faith is a wellness program administered by the Columbia Boone County Health and Human Services Department and through church networks. Live Well by Faith is designed to address the unique health related needs of African Americans in the Columbia community. Live Well by Faith is a program dedicated to decreasing the rates of diabetes and heart disease in the African American community of Columbia, Missouri.
